Our Mission
Our mission is to transform the hospice care experience for our patients and families in need of comfort and end-of-life support. We passionately commit to meeting the whole person with the level of services they need where they call home.
To meet more hospice patients and families early in their journey, Elevate Hospice… Transformed staff develops a plan of care for our patients and families based on individual needs. All of our services may be utilized throughout the plan of care as our patients’ needs change and we aggressively manage symptoms at the end of life.

Hospice Eligibility
Hospice care is an unlimited benefit accessible to patients with life-limiting illnesses. The patient should have a prognosis of six months or less if the disease were to take its normal course. Common diagnosis for hospice eligibility:
- Heart Disease
- Respiratory Disease
- Cancer Stage 4 or with Metastasis
- Neurological Diseases: Dementias, ALS, MS, Parkinson’s Disease
- Stroke or Coma
- Liver/Renal Failure
