Levels of Hospice Care
- Routine Care takes place in the home of our patients by the Hospice Team and is routinely provided based on the patients plan of care.
- General In-Patient Care is delivered in a setting with 24-hour RN coverage for acute symptom management.
- Continuous Home Care is provided in the home of the patient by Hospice-trained staff at the bedside for acute symptom management 8-24 hours a day for however long we need to manage the symptoms.
- Respite Care takes place in a skilled nursing community for caregiver challenges.
Bereavement Support
At Elevate Hospice… Transformed, we understand grief is a journey and no one should have to walk alone. Our General Bereavement and Monthly Grief Support Groups offer a safe, welcoming space for individuals who are grieving to share, listen, and find comfort among others who understand. Whether your loss is recent or long past, you are welcome here.
